Golden Buckeye Card Information

GOLDEN BUCKEYE CARD INFORMATION:

Participation in the Golden Buckeye program is free to all Ohio residents who are:

• Age 60 or over (proof of age required).

• Age 18-59 with a Social Security Administration defined disability.

If you are turning 60 and have an Ohio driver’s license, you will automatically be mailed a card just before your birthday. If you have not received the card four weeks after your birthday, or your card was lost or stolen, call 1-800-422-1976.
